2015 W2 and Form 1095-C Information 

All 2015 W2's will be postmarked by January 31, 2016 and mailed to your home address on file as of January 1, 2016. Current and Former Employees of P.F. Chang's and Pei Wei may print electronic copies of their 2015 W2. For more information please see below.

In 2016, some employees will receive a new tax form called the 1095-C that will contain detailed information about their health care coverage. It is important that employees keep this form for their records because they will need it to file taxes for 2015.

W-2 REPRINT AND RE-ISSUE INFORMATION

I did not receive my 2015 W-2. How can I get a copy?

2015 W2's will be postmarked by January 31, 2016 and mailed to the employee's home address on file as of January 1, 2016.

Current and Former Employees who know their employee ID number:

Current and former employees who know their employee ID number may access PeopleXpress to obtain an electronic copy of their W2. Log In at ess.pfcb.com, click "Myself" in the upper left corner, then choose W-2 under the "Pay" header.

Former Employees who do not know their employee ID Number:

Please go to www.mytaxform.com

  • Enter the 5-digit Employer Code: 12939
    • Enter your User ID which is your social security number (SSN)
    • Enter your PIN:
      • If you have never logged into the system, you will need to enter your Default PIN which will be the month and day of your birth followed by the last 4 digits of your social security number (MMDD####). You will then be prompted to enroll in the Authentication program. This step is designed to ensure secure access to your information.
      • If you have previously enrolled and logged into the system but have forgotten your PIN, please click on the "Forgot my PIN" link and follow instructions on how to obtain your current PIN.

Current and Former Employees who do not have access to a computer:

On or after February 15, 2016 please contact the P.F.C.B. Employee Line (877-424-2647, Option 8, Option 1) to report the concern. An agent will be available to assist you.

*Current or Former Employees should NOT contact the Restaurant Support Office (RSO) via phone regarding W2's. Phone calls received by the RSO regarding W-2's will not be accepted and employees will be redirected to the P.F.C.B Employee Line (877-424-2647).

Forgot your PeopleXpress Password?

If you do not remember your password, you may use the "Forgot your Password?" link to reset your password. This process requires a valid email address on file . If you receive a "Contact Your Administrator" error message this may be due to not having an email address on file in the system. To reset your password if you receive this message, please attempt to log in to PeopleXpress by typing in an incorrect password four times (which will lock you out of PeopleXpress). Your password will then be reset to the default password within 24 hours. The default password is the month and day of your birth and the last four digits of your social security number (MMDD####).

Are you a Current Employee and need a W2 from tax year 2011-2014?

Current employees may access PeopleXpress to obtain an electronic copy of their 2011-2014 W2's. Log In at ess.pfcb.com, click "Myself" in the upper left corner, then choose W-2 under the "Pay" header.

Are you a Former Employee who does not know your Employee ID Number and need a W2 from tax year 2011-2014?

Click Here to access the Request for Duplicate or Corrected Tax Form document. You will need to complete, sign the document in the presence of a notary and mail the document as directed to request a duplicate copy of your 2011-2014 W2. Please allow 1-2 weeks for processing.

Do you need to report a discrepancy with information as reported on your W2?

Please allow 1-2 weeks for processing.

1095-C Reprint and Reissue Information

What is a Form 1095-C?

The 1095-C contains detailed information about the health care coverage provided by your employer. If you were enrolled in health care coverage, worked an average of 30 or more hours per week, or were considered a full-time employee at any point in 2015, then you should receive a 1095-C. You will need it to complete your 2015 tax return. The information you report from the form will be used to determine whether you will pay a penalty for failing to have health care coverage as required by the Patient Protection and Affordable Care Act, also known as "Obamacare."

Can I get my P.F. Chang's/Pei Wei Form 1095-C Electronically?

Yes, if you did not receive your Form 1095-C via U.S. Mail, electronic reprint is available:
